1. Please provide us with details of the booking co-ordinator. This is who we will make up the contract for.

2. Address Of booking co-ordinator

3. Please provide the address of the venue in full.

4. Date of the event, arrival time, set up slot and any special instructions.

5. Changing facilities - is there an area the band can change/ use for breaks? If you have an onsite venue co-ordinator, please add their details here and we'll link in directly.

6. What food/refreshments will be available to the band? Please include a vegetarian option.

7. If you have a song request, please let us know the song choice and version. If you have a YouTube/ Spotify URL please include this. Request options: original track played through the band’s PA system, acoustic version of the song (guitar/ voice) or full band version of the track.

8. Pleae let us know if there's anything else we can do to make your day even more special.

Once we've received the form we will draw up a PDF contract and send this over for you to sign. Please note we are unable to confirm the band’s booking until receipt of the deposit (20%)
